RGN / Case Manager

New Horizons Care Ltd Case Manager/Registered Nurse We are currently recruiting for a Case Manager / Registered Nurse within the Leicester and Leicestershire area, supporting client?s with long term neurological conditions to live independently in the community. This role is predominantly 0900-1700 Monday to Friday however; some flexibility will be required to meet the demands of the role. You will be responsible for a group of client?s and their support teams offering regular supervisions and organising and attending team meetings. You will liaise with the Multi-Disciplinary teams and families supporting your clients. You will carry out full assessments of the client prior to setting up the package. You will produce accurate and detailed records and assist in the development of the service and carry out any other duties as required.
